It seems to me that there are two separate items to consider here, Andy, the actual photography and the editing.
The general camera work, I think, is quite good; but shooting this sort of scene without flash or other lighting aids is bound to create some lighting problems.
In some cases the background has excessively bright and distracting areas while there are deep shadows where you don't want them.
I would do a little extra editing either with Adjustment Layers and masks or two differently 'exposed' Raw conversions and combine them to produce a smoother and better balanced result.
Incidentally, what method did you use to convert to B&W?
