This pic shows the bag open with a 5DII and a 100 macro lens. There are four squarish slots for lenses and one on the right for another body (the 30D took the picture). In the slot for the extra body, I have both camera manuals.

This is a closeup at the top of the bag showing the original shoulder strap and the one I added. I had a shoe repair shop do the sewing.

This one shows where the new shoulder strap connects to the waist strap. The new strap is made by Lowepro - I was fortunate in that the shop where I deal had one in stock. Also note where the new strap "threads into" the original shoulder strap - this isn't necessary but it keeps the straps from flopping around.
In use, I undo the new strap at the buckle where it attaches to the waist strap - then it's just like the original and can be slid sideways for access.
If you are carrying a zoom on the main body, then the lens pocket that I presently store one lens in can be where the longer zooms fits. This still leaves enough room three lenses and a spare body (or two flashes and one lens).
In the space immediately above the main camera (first pic) I have three filter cases and one battery charger - the other charger is in the exterior pouch along with a remote release and some charts, etc.
