So how about if the photo caption represents a well known figure in another manner.
For example, Tony Blair stops by a rough hewn fishing boat in a thrid world country and someone takes his picture. That image is posted for sale and the caption indicates that he is a local villager standing by his fishing boat.
Quite explicitly, Blair is portrayed in a role that may be embarassing to him, or one that may hurt his career.
So, the image and the caption together? Does that make a difference?

I think the only way to get a reliable answer is to discuss a specific case with a local lawyer that knows this stuff.
Here we're not lawyers, and not in your jurisdiction, so it's easy to get advice that makes you do exactly the wrong thing...
