When you take an exposure that is that long, does the sensor get hot and generate noise that is sufficiently apparent that you have to deal with it during post-processing?

No - not at all.
When noise does raise it's ugly head after really long exposures (say 20 minutes plus) it tends to be in one of 2 forms ... either in the form of hot pixels (which ACR eliminates most of) (or I zap the few remaining ones manually) or in the form of shadow noise which in silhouette type shots is easily fixed with the blacks slider.
So either way, not a problem.
