Nice image. I would crop the building on the left for a better composition. Also, the lady would stand out more this way.

As Binnur suggested...the left building should go as well as cloning the draping rope thingy away.
Also, the title does display, to phrase it delicately...crass.

I find the three wheel assist interesting as I have only seen four wheel versions here.
With the title telling us to concentrate of the 'old lady' I would turn it into a vertical image by cropping the youth and the left doorway .. plus a horizontal flip would aid the resulting composition .... placing the dark area on right of frame. But that depends on if you wish a documentary or artistic image.

Putting the background out of focus I think subdues the interference of the rope or whatever it is ..partly cloned out over the brickwork but really too hard and complex a job over the background. The original would be better titled 'Old and Young' or something along those lines?
