The focus comments that others have made aside (as long as the eye closest to the camera is sharp, the softness you show in the other parts of the image are fine, and this seems to be the case (I think)).
1. I find that the eyes are getting to the "dark cavern" stage. Either a bit of fill light during the shot (preferred solution) or a bit of work in post can help that.
2. There is something strange and bright at the camera right eye. Something to be fixed in post.
Here is where I might go with those two small issues.
As for your light selection.
1. Beauty dish is a nice choice for this type of shot. Did you use a white or silver one and did you use a diffuser? I tend to be very close to my subject when I shoot a beauty disk (it sometimes gets into the frame and I end up having to crop it out).
2. No fill light? I virtually always either have a reflector or 2nd light as a fill light in this type of shot to push some light back into the face.
3. Hair light? Why. Your crop is so tight you really are not using it to separate your subject's hair / face / body from the background. I'm wondering why you decided to shoot with one when you really did not use it in the final composition.
