I don't understand what the title has to do with the photograph

I know a little about W. Eugene Smith ... his work was controversial, and he 'championed' the victims of Minimata disease.
The causes of spina bifida are not totally understood, possibly linked these days to folic acid deficiencies in pregrency, but I cannot see any connection to the titling of this image.

I like the image, but the title leaves me uncomfortable.... and confused.

I suspect that the poster is referring to the way that the shadowed spine divides the subject's back into two. He may well have been unaware of the medical condition. However, those who are aware of it are not unreasonably uncomfortable with the title (they include me).
