I've just sketched out the lighting setup used in this image using set.a.light 3D Studio.
The main (key) light is a white beauty dish above the subject. A light with a reflector, grid and snoot is set up at eye level to get light into the eyes behind the mask and a white reflector was used to fill in the shadows from below the model.
The shooting position was almost beside the fill light and parallel to it to get the eyes properly lit.
Excellent work Manfred this is the best of the three that I've seen of your series. A great mask and model.
Great image, I much prefer this to the one in your later post. There is a subtle lead in to the face and mask from the upper arm on the right and the firearm and hands on the left. I also think the eyes are better positioned within the mask. If it were mine (I wish!) I would spot heal the nodule on the skin on the bottom right hand corner to avoid the distraction. I also like the limited colour range which gives the image a particular coherence.

In general, I will only remove "temporary" blemishes like pimples or smudged makeup in this type of photography, unless the subject asks me to do more. This is more of a philosophical view than anything else and is one that I share with a lot of other photographers.
If this were a beauty shot going into a high end publication, I would be a lot more aggressive in my retouch work; assuming that this what the client wanted.
